The problem

A large manufacturing operation was managing accident and incident reporting through paper forms and email chains. A reported incident would be written up by a supervisor, handed to HR, passed to health and safety, and eventually filed — with no consistent process, no central record, and no way to track whether investigations had been completed or corrective actions implemented.

RIDDOR reportable incidents required manual extraction of information that should have been captured at the point of reporting. Management had no visibility of incident trends across the operation. Auditors reviewing health and safety compliance had to work through paper files.

What was built

A structured web application for end-to-end accident and incident management:

  • Incident reporting form capturing all required details at point of occurrence
  • Automatic classification by severity and type
  • Investigation workflow with assigned owner and completion tracking
  • Corrective action logging and follow-up tracking
  • RIDDOR reporting — automatic identification of reportable incidents with all required information pre-populated
  • Management dashboard showing incident frequency, type and location trends
  • Full audit trail of every report, investigation step and action taken
  • Role-based access for supervisors, H&S team and management

The outcome

Paper-based reporting eliminated. Investigation completion rates improved through structured workflow and ownership. RIDDOR reporting time reduced significantly. Management gained real-time visibility of incident trends for the first time. Compliance audits now completed against a searchable digital record rather than paper files.
